Keep a running list of medications that can easily change as the medications change. The list must include strength of pill, number of pills taken, and when taken.




Ask your doctor and pharmacist 


  1. A.What illness or condition is each medication treating?


  2. B.What are the side effects of each medication?


  3. C.Are there any interactions with other medications

  4. or foods?


  5. D.When and how is each medication taken, i.e.

  6. with or without food?




Ask your insurance carrier:


A. What is my out of pocket expense for each of the

medications?


B. What other options are there to reduce my

out of pocket expense?



Ask drug company:


Are there vouchers or assistance programs

available to lower the cost of medications?


Ask pharmacist, drug company, national foundation, or other support group:


Are there are any financial assistance programs or vouchers available to lower drug costs?
